Client’s , hereby acknowledge and agree to the terms and conditions outlined below in connection with the services provided by Viridian Healing - Mind & Soul ​Pathways ("Provider"):

Implied Consent: By engaging in counseling sessions and energetic healing practices with the Provider, I give my implied consent for the services rendered, ​understanding that they may involve discussions of personal matters and physical touch as part of certain modalities.

Duty of Care: The Provider agrees to exercise reasonable care and skill in providing services. However, I understand that the results of counseling and energetic ​healing practices may vary and are not guaranteed.

No Liability Clause: I acknowledge that the Provider is not liable for any injury, loss, or damage arising from participation in counseling sessions and energetic ​healing practices. I understand that it is my responsibility to consult with a qualified healthcare professional regarding any medical or psychological concerns.

Parent/Guardian Presence: I understand that for sessions involving children, a parent or guardian must be present during the session. Standard reporting ​procedures will be followed for suspected harm to children.

Payment Terms: I agree to make payments for services rendered at the time of the session. No credit will be extended for services rendered.

Touch Involvement: I acknowledge that certain modalities used by the Provider may involve physical touch as part of the healing process. I consent to such touch ​as deemed appropriate by the Provider.

Withdrawal of Consent: I understand that I have the right to withdraw my consent for any part of the services provided at any time. I understand that ​withdrawing consent may affect the effectiveness of the services and I may discuss alternative options with the Provider.

48-Hour Cancellation Fee: I understand that a 48-hour cancellation notice is required for Quantum Healing Hypnosis Technique (QHHT) sessions. Failure to provide ​adequate notice may result in a cancellation fee.

Recording Policy: I acknowledge that all quantum Energetic sessions are recorded for quality assurance and professional development purposes. These ​recordings are confidential and will not be shared without my explicit consent, except as required by law. Clients may have ACCESS TO a copy of the recorded ​session.

Private Information Collection: I understand that the Provider may collect and store personal information as part of providing counseling services. This ​information will be kept confidential and will only be used for the purpose of providing services. No Medical nor Judicial Reports will be provided to a thrid party.

I acknowledge that I have read, understood, and agree to the terms and conditions outlined in this Liability and Waiver Form.
